Fantasy 5 is a lottery game played in the state of Michigan. You have to choose five numbers between 1 and 39, and win prizes for matching two or more numbers with those drawn. Fantasy 5 costs $1 per entry.
You win the Fantasy 5 jackpot if you match all five of the numbers that are drawn. The top prize, which is always paid as a lump sum, starts at $100,000 and rolls over if it is not won.
For an additional $1 stake, you can play the Fantasy 5 EZmatch add-on for a chance to win up to $500 in instant cash prizes if one of your EZmatch numbers matches one of your Fantasy 5 numbers.
You can also add the Double Play option to your Fantasy 5 play for an extra $1 to enter your numbers in a separate Double Play draw with a top payout of $110,000.

Double Play
In Fantasy 5, you also have the option to pay an additional $1 to opt in to Double Play. A second set of winning numbers is drawn after the main Fantasy 5 drawing, and you can win by matching the numbers on your ticket with the winning line. If there are multiple tickets that match all the Double Play numbers drawn, the top prize is shared equally by all winners.
Double Play uses the same number range as the main Fantasy 5 draw, so the odds of winning are the same. When you play, you also use the same numbers for both draws - you don't pick another set. It is therefore possible for a particular number to appear in both draws on the same night, and you can win prizes in both the regular and Double Play draw.
Non-jackpot Double Play prizes are twice the value of the prizes in the regular draw, and the jackpot is fixed at $110,000.
EZmatch
EZmatch is an add-on to the main Fantasy 5 game that awards instant cash prizes if any of your chosen numbers match the random EZmatch numbers printed on the lower portion of your ticket. Each EZmatch number is assigned a cash value between $2 and $500. If any of the EZmatch numbers match those that you chose for the main draw, you win the corresponding cash prize. EZmatch costs an additional $1 per play.

How to Play Michigan Fantasy 5
You can play Fantasy 5 online or by buying tickets from authorised retailers across Michigan. Follow the steps below to play Fantasy 5:
- Select five numbers between 1 and 39. Alternatively, choose the Easy Pick option to have your numbers randomly selected
- Select whether you want to play EZmatch for an additional $1 (in-store only)
- Mark the Double Play box if you want to participate for an extra $1 (in-store only)
- Choose how many draws you want to enter in advance, up to a maximum of 60
- Pay for your ticket with the retailer/online provider
- If you play EZmatch, check the numbers on the lower part of your ticket to see if you’ve won. You can claim EZmatch prizes straight away
- Keep your Fantasy 5 ticket safe and check the results of the draw to see if you have won

Your prize will be deposited directly into your online account and may be withdrawn at any time.
$601 to $99,999.99
If you played on the Michigan Lottery's website, you'll need to make an online claim. Log in and go to My Account and then Tax Reported Prizes. Follow the steps to claim. You'll need to submit copies of your Social Security card and government-issued photo ID such as a driver's license. After processing, the prize will be paid into your Lottery account and may be withdrawn at any time.
$100,000 and Above
Make an online claim and then visit Michigan Lottery headquarters at 101 E. Hillsdale in Lansing to claim your prize. To begin the claim, log in and go to My Account and then Tax Reported Prizes. Follow the steps to claim.
Before you visit Lottery headquarters, wait to receive an email from the Lottery with instructions to call Lottery Player Relations to make an appointment.
When you visit Lottery headquarters, you'll need your original Social Security card and government-issued ID, such as a driver's license.
How to Claim a Prize from a Retail Ticket
Here's how to redeem a winning ticket you purchased at a Lottery retailer.
$600 and Below
Tickets worth less than $600 may be cashed out at any Lottery retailer.
You can also claim by mail. Complete a Ticket Receipt Form or make a written request for payment including your full name, address, and phone number. All claims must include the signed ticket plus a copy of your Social Security card and a government-issued photo ID. For security, keep a copy of the ticket and form and send your claim by registered mail to:

To claim in person at a Lottery Regional Office, please call 1-844-917-6325 to make an appointment. You'll need to show the original ticket, Social Security card, and government-issued photo ID.
If you visit a claim center without an appointment, you have the option to drop off your documents.
You can also claim prizes from $600 up to $99,999.99 by mail. Complete a Ticket Receipt Form if you are the only ticket-holder, or a Substitute 5754 Form for club prizes.
All claims must include the form, signed ticket, and copies of your Social Security card and a government-issued photo ID. For security, keep a copy of the form and ticket and send your claim by registered mail. Mail documents to the Lottery at the address above.

Claim via any Lottery regional office from 7:45 AM to 4:30 PM Monday through Friday.
